What To Bring
Preparing ahead of time can help your visit or admission go more smoothly. What you need to bring depends on the type of services you are accessing at ACCA. Please review the section that applies to you. If you are unsure about an item or whether it is required, check with your specific ACCA location before your visit.
Outpatient Assessments and Admissions

Bring the items listed below to your scheduled outpatient assessment or appointment. Missing information may result in your appointment being rescheduled.
Identification and Insurance

Photo ID (Driver’s License, Non-Driver ID, or Passport)

Copy of insurance card(s)

Insurance or Medicaid card
-
Any required co-payment or appropriate fees
Medicaid or Social Services Documentation
Bring one of the following, if applicable:

Letter from the Department of Social Services stating they will pay for assessment services

Proof that you have applied for Medicaid and are waiting for approval
If you are Medicaid pending and later denied, or if required paperwork is not completed, you will be responsible for any balances accrued. If you are Medicaid eligible and have not yet applied, you should do so before attending your appointment. Proof of Medicaid application for benefits will be required at time of visit.
Medical and Treatment Information

Copy of your most recent physical exam from your primary care provider

Dates and locations of any prior or current treatment
Legal or Referral Information (if applicable)

Information related to current or past legal issues

Names and phone numbers of legal representatives, law guardians, or courts assigned to you or your children

If referred through the Impaired Driving Program or a related referral:

Driving Abstract from Motor Vehicle

Arrest record including dates of violations and BAC (blood alcohol concentration) levels

Important Information to Know

Appointments may be rescheduled if required payment or documentation is not available.

If you have private insurance, you must contact your provider before your appointment to obtain any required pre-authorization. If you do not pre-certify, you may be responsible for all charges.

Individuals under 18 must be accompanied by a parent or legal guardian.

Assessments typically take 1 to 2 hours.

If prior payment arrangements were made with the front desk or Finance Manager, be prepared to pay the agreed-upon amount at your appointment.

ACCA does not accept Medicare or MassHealth.
