ABOUT P.A.T.H.
The Addictions Care Center of Albany’s P.A.T.H. (People Advancing through Treatment and Housing) Program provides outreach, intensive case management and integrated treatment for chronically homeless men and women who have substance abuse disorders and co-occurring mental disorders.
HOW IT WORKS
ELIGIBILITY
Self-referral, referral from a family member, another licensed agency, social service agency or governmental source for those who are homeless, chronic substance abusers or experience mental health or trauma issues will allow for admittance into the P.A.T.H. Program.
NEXT STEPS
ACCA staff will review all pertinent records for the potential resident and schedule a face-to-face assessment and screening. Individuals who are admitted into the P.A.T.H. Program will meet with a Psychiatric Nurse Practitioner, Intensive Case Manager, and Co-occurring Counselor to develop a comprehensive Service Plan to address all pertinent life areas.

• Chemical Dependency Treatment

• Intensive Case Management
• Support services through community providers as needed
